A Minimally Invasive Approach
Nothing ages a smile and face more than receding gums. Gum recession can also weaken teeth, causing sensitivity, root damage or even tooth loss. With the new Pinhole Surgery Technique (PST), receding gums can be repaired without the use of traditional surgery that includes flapping open the gum. Dr. Hendry and Dr. Henriod are currently among only a handful of doctors in the United States offering this breakthrough treatment. The PST gum recession procedure has been tested over a period of several years with good results.
With the new Pinhole Surgical Technique, we can treat all the affected teeth in one session, although patients can also opt for treating just one arch or quadrant at a time. Treatment is done using local anesthetic and treatment time is only one to two hours. Patients typically have needed only over the counter pain medicine post treatment. Results are stable, with cases maintaining their gum coverage for years.
How It Works?
Unlike soft-tissue grafting, the Pinhole Surgical Technique begins by creating a pinhole in the gum tissue that is receding. Once the pinhole has been created, a device is placed into the hole. This device peels and stretches the gum into a healthier and more suitable position on the tooth (the gums are brought downward). Once the gum tissue is in the desired position, Dr. Henriod or Hendry will insert collagen strips into the pinhole in the gums. While soft-tissue grafting uses stitches to keep the gums in place, these strips keep the gums in place in a much more minimally invasive way and help to create new tissue that will ultimately secure the gums in their new location.
The affected teeth are cleaned and prepared and the gum tissue is numbed with a local anesthetic. A small entry point (0.1 inch) is made in the gum above the tooth or teeth to be treated.
Drs. Hendry and Henriod use a specialized dental instrument designed and patented by PST inventor Dr. Chao to free the gum tissue at the entry point and gently move the gums down to their proper position. Next, they use a small angled instrument to pass several collagen strips or gum graft material through the entry point and place them under the gums. Small sutures may be used to stabilize the gum position.

GINGIVAL RECESSION CAUSES
Gingival recession is, essentially, the technical term for when gum tissue deteriorates and exposes more of the tooth. Problems arise because the more that the gums recede, the higher the likelihood is that the tooth’s root will be exposed. This creates pain, sensitivity, and over time can result in the loss of the teeth affected. There are numerous reasons why gums recede. Genetics play a big role in determining susceptibility to things like periodontal disease which is the leading cause of gum recession. Aside from periodontal disease, gingival recession can also be caused by the lack of proper dental care. Adversely, it can be caused by too much brushing of the teeth or by brushing the teeth in a forceful, extreme manner.

Am I A Good Candidate For The Pinhole Surgical Technique?
When considering the Pinhole Surgical Technique, it is important to determine whether you are a good candidate for the procedure. Your dentist or periodontist will have the final say, but there are certain factors that usually make or break a person’s candidacy for the Pinhole Surgical Technique.
You are likely a good candidate if:
- Your receding gums are more of an aesthetic issue than they are a dental health issue, the Pinhole Surgical Technique is a viable and worthwhile option.
- You do not smoke or vow to quit smoking.
- You are in good health; this applies to your dental health as well.
However you will likely not be a good candidate if:
- You have been diagnosed with periodontal disease
- You do not have an ample amount of gum tissue to use for the technique, a certain amount of gum tissue is required for the technique to be successful.
WHAT YOU SHOULD KNOW ABOUT PINHOLE SURGICAL TECHNIQUE FOR GUM RECESSION?

Pinhole is one of many successful techniques used in our practice! With the Pinhole Surgical Technique, we can treat all the affected teeth in one session, although patients can also opt for treating just one arch or quadrant at a time. This approach allows Dr. Henriod and Hendry to customize the treatment to each individual patient and tooth.
Collagen is used in the Pinhole Technique. Collagen is the main structural protein found in fibrous tissue such as skin, ligaments and bone. It provides elasticity and strength. The Pinhole Technique does not require a tissue graft other than the use of collagen. The patient’s existing gum tissue is repositioned (not cut) to cover the exposed tooth roots and collagen is used to stabilize the gums in their new position.
Frequently Asked Questions About Pinhole Surgical Technique
How long do the results of the Pinhole Surgical Technique typically last?
When performed by an experienced periodontist, results from PST can remain stable for many years. Longevity depends on proper oral hygiene, managing contributing factors like grinding or misalignment, and attending regular professional cleanings. Once the gum tissue has healed and stabilized, most patients enjoy long-term root coverage without needing additional surgical intervention.
Is the Pinhole Technique painful compared to traditional gum grafting?
Most patients describe PST as surprisingly comfortable because it avoids incisions and sutures. Only a small entry point is created, which reduces postoperative soreness. While mild tenderness is normal for a day or two, discomfort is generally minimal and easily managed with over-the-counter medication. Patients commonly report returning to normal activities more quickly than with conventional grafting procedures.
Can the Pinhole Technique treat recession caused by orthodontics or grinding?
Yes. Many patients experience gum recession due to past orthodontic movement, nighttime grinding, or bite imbalances. PST is often a suitable option for these cases because it repositions the existing tissue rather than requiring a tissue graft. During your evaluation, the doctor will also recommend complementary treatments, such as nightguards or bite adjustments, to help prevent future recession.
Is the Pinhole Surgical Technique safe for older adults?
Absolutely! PST is often ideal for older adults because it is minimally invasive, requires no surgical graft harvesting, and involves a shorter healing period. Many mature patients appreciate that the tissue looks natural and that the procedure can address several areas at once. If overall gum health is stable and there is sufficient tissue to reposition, PST can be an excellent solution.
How much does the Pinhole Surgical Technique cost?
Cost varies depending on how many teeth require treatment and whether additional therapies are needed to support long-term stability. As PST can treat several teeth in one visit, patients often find it more cost-efficient than traditional grafting. After your consultation, you’ll receive a personalized treatment estimate, so you clearly understand the investment before scheduling.
Does insurance cover the Pinhole Surgical Technique?
Insurance coverage differs between plans, but many carriers provide benefits for treating gum recession when it impacts oral health, causes sensitivity, or exposes the roots. As PST is an alternative to grafting, not cosmetic surgery, many patients receive partial coverage. The Pasadena Periodontics team reviews each patient’s benefits and submits documentation to maximize reimbursement.
How do I know if PST or traditional gum grafting is better for my situation?
While PST is highly effective, not every case is suited for it. Patients with significant active periodontal disease or extremely thin tissue may achieve more predictable results with alternative grafting. During your consultation, the doctor evaluates gum thickness, attachment levels, bite forces, and recession severity to match you with the safest, most stable option for long-term success.
Can the Pinhole Technique be combined with other periodontal treatments?
Yes. Many patients benefit from combining PST with scaling and root planing, bite correction, or collagen-based regenerative therapy to protect their results. Treating underlying inflammation or biomechanical issues improves long-term stability. Your treatment plan will highlight whether completing these procedures before or after PST will produce the best outcome.
How can I find a qualified provider near Pasadena for the Pinhole Technique?
PST is a specialized treatment taught to a limited number of clinicians. Dr. Hendry and Dr. Henriod are among the earliest certified providers and have extensive experience in performing the procedure. Patients from Pasadena, Arcadia, Glendale, Altadena, and the greater Los Angeles area routinely seek out their expertise because of their advanced microsurgical approach and predictable results.
Are appointments available for patients traveling from outside Southern California?
Yes. Patients often travel for PST because few specialists nationwide are extensively trained in the technique. Pasadena Periodontics can coordinate consultation and treatment visits for out-of-town patients, helping minimize the number of trips required. If you live outside Pasadena, or even outside California, you can contact the office to discuss scheduling options.
Advantages To The Pinhole Surgical Technique
MORE EFFICIENT THAN TRADITIONAL TREATMENT
One of the main reasons patients opt for the Pinhole Surgical Technique is because this technique can address multiple locations in one sitting. The Pinhole Surgical Technique allows a patient to have their full mouth treated at once which saves them time and money in the long run.
SHORTER HEALING PROCESS
Another advantage to the Pinhole Surgical Technique is that the healing process is swift and efficient, so you spend less time in recovery.
